YAT-Therapy & Wellness

Comprehensive Therapy in Your Home or Clinic

Our Mission

Our mission is to help adults live with greater independence, safety, confidence, and quality of life through personalized physical, occupational, and speech therapy. We provide collaborative care in our Martinez clinic and throughout the community, meeting each person where they are and adapting their care as needs change—from restorative rehabilitation to maintenance and quality-of-life support.

Our Vision

Our vision is to be the CSRA’s trusted destination for accessible, relationship-centered rehabilitation across the continuum of care. We envision a community where adults and their families can receive coordinated therapy in the setting that best supports their goals, remain engaged in meaningful activities, and have a knowledgeable care partner as their needs evolve.

Our Values

  • Client Commitment: We strive to provide the best services to assist clients in achieving their personal goals. We strive to go above and beyond for our clients.

  • Quality: Our therapists provide outstanding services utilizing evidence to support treatment provided to allow clients to get the best results possible.

  • Integrity: Our therapists do what is right and what is best for the client while being ethical, honest, open and genuine.

  • Passion: We are committed to our clients and are passionate about working with all our clients.

Therapy Build Around Real Life

Therapy should fit into your life—not the other way around. Our flexible model allows us to provide physical, occupational, and speech therapy in our Martinez clinic or in the places where daily life happens, including homes and senior living communities.

Care in the setting that works for you
Patients may receive therapy in our clinic, at home, or through a combination of both. Patients do not have to be homebound to benefit from our mobile outpatient services.

One collaborative therapy team
Our physical, occupational, and speech therapy professionals work together to address mobility, daily activities, communication, cognition, swallowing, safety, and independence.

Goals that are meaningful and practical
We look beyond isolated exercises and focus on what each person wants and needs to do in everyday life—from safely navigating the home to returning to community activities.

Support as needs change
Our services can extend beyond traditional restorative rehabilitation. We also offer private-pay and maintenance options and can collaborate with hospice providers to support comfort, function, caregiver education, and quality of life when appropriate.

A personal, relationship-centered approach
As a locally owned practice, we take time to understand each patient’s priorities and include families, caregivers, and healthcare providers in the care process.
